What sets us apart is the combination of our passion for reducing health inequalities through making differences to people’s lives coupled with the direct experience of recruiting, training, and hosting staff under the NHS Additional Role Reimbursement Scheme (ARRS). This unique experience has directly informed the development of the Personalised Care Training Academy, the development of the PCI accredited training programmes and our innovative consultancy services. Our training has resulted in improvements in retention, resilience, role satisfaction and competence aswell as clarity regarding how the new ARRS roles fit together, to create a coherent and effective, person-centred approach
Our curriculum is designed to deliver more than just training, but also measurable skills and knowledge that health and social care staff need to support the implementation of the NHS Long Term Plan and Comprehensive Model for Personalised Care thus, ensuring personalised care becomes ‘business as usual’

Since our establishment, the Personalised Care Training Academy has been supporting multiple healthcare providers, including ICBs, PCNs, surgeries and other organisations throughout the country to integrate the comprehensive model of Personalised Care into their everyday practice. Meet the team
Josh Matthews
Operations Manager
Josh leads the development of personalised care training programmes within Pathways CIC and plays a central role in embedding personalised care approaches into practice. He brings a strong background in physical activity, behaviour change and service leadership across health and wellbeing settings.
Holding a BSc in Physical Activity and Health and qualified as a Level 3 Exercise Referral Specialist, Josh has led the design, delivery and evaluation of exercise referral and work and health services. He has managed multidisciplinary clinical teams supporting people with health conditions to move closer to employment. As Operations Manager for the Personalised Care Training Academy, Josh ensures training is practical, high quality and grounded in real-world delivery experience.
Lorraine Whitworth
Personalised Care Training & Development Facilitator
Lorraine is an experienced trainer with a strong background as a Pathways Health Coach, bringing first-hand experience of delivering personalised care in practice. She is passionate about sharing her knowledge and real-world insight to support the integration of personalised care into everyday roles.
As part of a training team that also acts as a host employer for personalised care roles, Lorraine brings valuable understanding of the three key personalised care positions, how they work together, and the challenges commonly faced by those new to the role. This perspective enables her to deliver training that is practical, relevant and firmly grounded in real practice experience.
Lorraine embodies the core values of personalised care — empathy, understanding, kindness and a non-judgemental approach, creating a supportive and inclusive learning environment.
Alex Potts
Operations Manager
Alex is passionate about people development and bringing the principles of personalised care to life. She is committed to supporting individuals to flourish within their roles by building confidence, capability and meaningful practice.
Through her work, Alex contributes to the delivery of training that is grounded in personalised care values and focused on real-world application, helping learners translate theory into effective, person-centred practice.
Dr Ihtesham Sabri
Clinical Director Pathways CIC
Ihtesham is a passionate advocate for mental health, with extensive experience across clinical practice, education and leadership. During the COVID-19 pandemic, he contributed to the frontline response and continues to speak nationally and internationally on mental wellbeing, human rights and healthcare equity.
He studied medicine at St George’s, University of London, training in both psychiatry and general practice, and has held senior roles including Senior Partner in a London GP practice, GP Postgraduate Tutor for the Kent, Surrey and Sussex Deanery, and Chair of the Royal College of GPs South-East Faculty. Ihtesham currently works within the NHS and serves as Clinical Director for Pathways CIC, bringing strong clinical and educational expertise to the development of high-quality training programmes.
Yvonne Clarke
Founder & Managing Director Pathways Community Interest Company and the Personalised Care Training Academy
Yvonne Clarke MBE is a nationally recognised leader in personalised care, work and health, and workforce development, with over 25 years’ experience designing and delivering high-quality, person-centred services across the NHS, local government and the voluntary, community, faith and social enterprise sector.
As Founder and Managing Director of Pathways CIC, Yvonne has led the development of accredited personalised care, social prescribing and integrated work and health services operating across multiple Integrated Care Systems. Awarded an MBE in 2020 for services to work, health and wellbeing, she brings deep system insight and hands-on delivery experience to ensure training is practical, evidence-based and aligned to national policy and real-world practice.

Pathways Community Interest Company
The Personalised Care Training Academy is the training arm of Pathways CIC, an award-winning, forward-thinking, innovative Social Enterprise operating since 2006.
Pathways CIC delivers a range of pioneering health, work and wellbeing programmes designed to create sustainable change for individuals and local communities, focusing on enabling people to live happier and healthier lives.
Pathways CIC has direct experience of recruiting, training and hosting staff under the NHS Additional Role Reimbursement Scheme (ARRS) and has staff supporting behaviour change through health coaching techniques on a wide variety of public sector contracts. This experience has informed the development of the Personalised Care in Practice – training programmes.
Pathways CIC is an ISO 9001 accredited organisation, therefore the PCTA seeks to continuously improve and gain feedback relating to each training module from participants to facilitate the continuous improvement of the training programme. Regular reviews will be undertaken to review the impact and effectiveness of the training provided.
